TISD Secondary Insurance Policy
Temple ISD has purchased a SECONDARY insurance policy for all student athletes at Temple High School. All insurance claims for athletic injuries on this policy must be completed through the athletic training room. We ask that all non-emergency athletic injuries are seen in the athletic training room prior to any physician visits in hopes of relieving you of any additional, extra physician costs.
The form will be filled out by a school district official with the information about your son/daughter’s injury. Please complete the bottom of the form labeled Other Information. Once filled out, this form and any other bills or questions associated with it will need to go through the insurance company. There are directions on the back of the form on how to file a claim.
We suggest making a few copies of this form and keeping the original. You can then use the copies to send in with any bills etc.
