If you need to request a copy of your child Physical or medical paperwork, see the options below:
1. Login to the Rank One parent portal, download a copy for your records
2. Have school contact Temple AT requesting a copy. Email: windee.skrabanek@tisd.org
3. Complete the Medical Records Release Form document, sign and return.
